Short description of the contract or purchase(s): Medway Council is seeking to commission a continuum of service provision for young people in Medway to meet their individual needs at each stage of them reaching independence. Medway Council is seeking to commission services for the following young people:
– Accommodation and support for young people who are Looked After/Eligible and Relevant children – aged 16 to 18 years of age. There may be the occasion where young people will remain in supported accommodation post 18 years of age, where all parties agree that it is in the best interests of the young person to do so.
– Accommodation and support for Care Leavers/Former Relevant Children aged 18-21 years or until they finish their education/training programme (24 years) or up to 25 years if they are a disabled young person. Young people should be accommodated in good quality and appropriate housing that will accept housing benefit when they turn 18 years of age.
– Young people 16-17 years of age who present as homeless and require immediate accommodation and/or support whilst awaiting a Joint Housing Assessment to be undertaken. As well as accommodation thereafter. Floating support may be required dependent on the young person’s assessed needs.
The following type of accommodation and support arrangements will be required:
– Supported Accommodation/Group Living Arrangements: This could comprise of communal/shared accommodation that can house a small number of young people, enabling them to live together. The home must be managed by highly trained, experienced and skilled staff, some of whom are to be on site 24 hours. Support staff are to provide appropriate levels of support designed to meet young people’s individual needs and achieve the outcomes in the service specification and the young person’s pathway plan.
– There may be on occasion the need for a young person to be accommodated in a solo placement due the severity of their needs with very specialist and intensive support provided.
– Accommodation with Floating Support/Supported Tenancy Arrangement: Medway is seeking providers to source accommodation and provide appropriate levels of support that will enable young people to enter and maintain their own tenancies. Housing must be sourced through providers own stock or arrangements with private or social landlords. This could be in the form of one bed flats, shared flats, bedsits within a larger communal buildings or trainer flats as an example. Flexible packages of floating support are to be offered, which may vary from a couple of hours a week to higher levels of support e.g. 7-8 hours a day, 7 days a week dependent on the young person’s assessed need. It is anticipated that the level of support will decrease over a period of time, as the young person develops their ability to live independently.
– Floating Support/Outreach only: This could be community based floating support to enable young people to live semi independently in their own accommodation. We are seeking to commission flexible packages of support which may vary from a couple of hours a day to a few hours a week.
