The current contract for Health and social work services – Recurrent Lead is projected to end on 28-MAR-21 and may be available for retender. Arrangement of Care (Brokerage).

The Provider will promote independent living by offering a range of advice and support services to people over the age of 18, their Carer(s) (including parent(s)) and/or a nominated person where a person lacks mental capacity or is under the age of 18. The specific purpose is to ensure that individuals are able to make an informed choice about their social care and/or health services, or other relevant services. This will include the provider making their advice and support services available to anyone who has a support and/or care need, the individual’s representative or individuals who self-fund their support needs.
