Justification for a framework agreement, the duration of which exceeds four years: The framework agreement is for an initial period of 2 years with an option to extend (subject to organisational need, budget availability and performance) extended on the same terms and conditions at the Council’s option for a further period or periods of up to 2 years, followed by a further option for the Council to extend for a further period or period of up to 2 years, with extensions not exceeding 4 years in total, the maximum duration of the New Framework will therefore be 6 years from the commencement date of the New Framework. This is a framework for Part B Services as defined by the Public Contracts Regulations 2006 and therefore the maximum 4 year period for a framework does not apply.

Short description of the contract or purchase(s): Framework Agreement for the Provision of Care and Support at Home Services.
The procurement covers the delivery of the Services to residents entitled to such a service either from the Council or from the CCGs.
The Commissioners want to ensure that, whenever possible, adults receive health and social care in their own homes in a way that is personalised, with a strong focus on individual well-being, choice and control.
Providers appointed to the New Framework will support this aim through the provision of the Services.
The following services are included:
Maintaining Independence and Urgent Care – Care and Support at Home funded by the Council and the NHS.
Maintaining independence services are expected to be enabling in approach to support people to optimise their independence in areas where they are able, to maintain the levels of independence in areas they are not able to develop and to prevent further care needs from developing in other areas where possible.
Including the provision of basic activities of daily living (for example personal hygiene, dressing and preparing food) and social, practical and emotional support to meet the identified assessed needs and outcomes of people using the service.
Includes maintaining independence services provided on an urgent basis, with commencement of service within 3 hours from confirmation of allocation, and delivery of service for a maximum of 2 weeks.
Fast Track End of Life.
The provision of personal, social, practical and emotional support for patients at end of life who have been assessed as being eligible for NHS Continuing Health Care (NHS CHC) ‘Fast Track’ to support the person to die at home, as part of an agreed Advance Care Plan.
Reablement.
Reablement is a personalised and proactive approach that supports people to maintain, retain or regain skills and abilities following a change in circumstances, illness, injury or crisis.
The service will be provided in an enabling way to optimise the independence of adults using the service, in order that they maintain social contact and their quality of life in an environment that is supportive and safe, yet engenders development and the opportunity for positive risk taking.
In addition to the services outlined, Maintaining Independence and Urgent Care – Care and Support at Home, and Fast Track End of Life services may include Live In Care provision as outlined in paragraphs 8.20-8.25 of the draft Specification.
