Bournemouth Borough Council is seeking to commission Care and Support Services for any person with a Learning Disability and/or Autism living at Fairways Court who is entitled to receive services under The Care Act 2014 or through joint funding under NHS Section 117 funding arrangements with NHS Dorset Clinical Commissioning Group.
The Provider will be required to provide person centred care and support service for adults aged 18 years and over, or who are aged 16 and over, coming through transition into adult services, with a Learning Disability and/or Autism who are ordinarily resident in, or are the responsibility of, the Bournemouth Borough Council.
Fairways Court consists of 6, 1-bedroom apartments across 2 floors, in the BH11 area of Bournemouth and forms part of the Council’s Transforming Care Partnership Plan for Individuals with learning disabilities and/or autism.
