Intermediate Care Services in Barnsley are provided as part of a formal Alliance Contract which consists of Barnsley Clinical Commissioning Group, South West Yorkshire Partnership Foundation Trust (SWYPFT), Barnsley Hospital NHS Foundation Trust, Barnsley Healthcare Federation and BMBC. Intermediate Care is part of the New Models of Care service delivery within the Barnsley geographical footprint.
We are seeking Care Home Provider(s) to provide care beds, including the ability to accommodate nursing and EMI when required, for patients who require 24-hour supervision during their rehabilitation programme. Up to 48 beds are required; ideally located in 2-4 units spread across the borough (i.e. likely to be more than one provider). This may include Care Home Provider(s) close to Barnsley’s geographical borders.
