United Kingdom-Hounslow: Health and social work services – Framework Lead

The London Borough of Hounslow is recommissioning its health and well being service. The aim of this is to introduce a high-quality integrated health and well being service that meets the needs of the local population, is person centred and builds on community assets to improve the health and well being of our population and reduce health inequalities.
The service will predominantly cover the following lifestyle factors: smoking, inactivity, healthy weight (including child weight management) and modifiable risk factors for cardiovascular disease. The service will also be expected to provide a broad range of advice, information and support to access other local services and activities that improve people’s well being as well as addressing social isolation. The service will need to make use of both modern technology and more traditional communication methods.
The Authority would like to invite qualified and experienced providers to bid for this service for adults and children as a single service provider.
The Contract will be for an initial period of 3 years commencing in April 2016 with an option of a further 2 years in 12-month periods at the sole discretion of the Council. The total contract value is up to 1 000 000 GBP per annum. Year 1 of the contract will be a block payment with subsequent years comprising a 60 % block payment and 40 % of the payment based on activity.
