The current contract for Health services – Framework Lead is projected to end on 13-FEB-19 and may be available for retender. HQIP NCA 176 National Retrospective Case Record Review Programme.
The overarching aim of the project is to establish and roll-out a standardised methodology and process for retrospective case record review (RCRR) for adult, acute care deaths in England and Scotland to improve understanding and learning about problems in care that may have contributed to a patient’s death
This work is not designed to generate data for comparison of Trust performance or to contribute to a national measure of the number of deaths due to problems in care. A national estimate of the number of deaths due to problems in care will be generated via Lot 2 of this specification. The data generated from this programme is primarily for use by Trusts to support their own learning and improvement.
This contract opportunity is being offered in 2 lots as follows:
Lot 1: National Retrospective Case Record Review Programme
The overarching requirements of this Programme are as follows:
– To establish a single standardised mortality review process across all acute care hospitals in England and Scotland. The pro forma for case assessment should be based on that developed by the Preventable Incidents, Survival and Mortality Study (PRISM) study.
– To design and deliver training to acute hospital trusts as they adopt the newly developed mortality retrospective case record review process.
– To make available an accessible website to Trusts to provide information and access to all supporting materials and provide support for Trusts to coordinate, collect and collate the outputs of their local reviews.
– To integrate this standardised process within acute hospital trusts’ wider clinical governance systems, ensuring deaths thought to be due to problems in healthcare, are reported to local risk management systems with appropriate identifiers and uploaded to the National Reporting and Learning System (NRLS) to facilitate national learning.
– To provide support to acute hospital trusts to systematically review their local systems, processes and organisational culture and enable improved learning from these reviews to drive improvement in the quality of care delivered to patients and reduce the occurrence of avoidable harm.
– To engage with patients, families and carers, healthcare professionals, commissioners and regulators to support high quality local reporting of outcomes and learning from case record reviews.
– To produce an annual report describing programme progress.

Lot 2: National Programme to use retrospective case record review to estimate the proportion of avoidable hospital deaths in English acute NHS Trusts
The overarching requirements of this Programme are as follows:
– Using a sample of case records from patients who have died in English NHS acute care hospital Trusts, to undertake retrospective case record reviews (RCRR) using PRISM 2 methodology on an annual basis, in order to estimate the overall proportion of avoidable deaths (i.e. resulting from acts of omission and commission) and produce a single indicator.
– To publically report that indicator in the NHS Outcomes Framework on an annual basis.
– To use robust statistical methods to assess trends over a period of several years. Data must be able to distinguish a reasonable change in the rate of avoidable hospital deaths over time.
– To engage with service users and their families, healthcare professionals, commissioners and regulators to consider and plan the national reporting of emerging themes and subsequent recommendations from this national case record review process.
Suitable providers for Lot 1 will need to be able to demonstrate:
– Effective and fully-integrated clinical leadership.
– The ability to engage actively with all relevant professional disciplines.
– Robust project management skills.
– Methodological expertise and experience, including experience of conducting case record reviews and understanding case record review methodology.
– Experience of developing and implementing a training programme to support implementation of change.
– Sound economic and financial standing assessed by evidence of professional indemnity insurance and audited accounts covering at least three previous financial years.
Suitable providers for Lot 2 will need to be able to demonstrate:
– The ability to engage actively with all relevant professional disciplines.
– Robust project management skills.
– Methodological expertise and experience, including experience of conducting case record reviews and understanding case record review methodology, and statistical analysis.
– Experience of managing information governance issues and obtaining the necessary permissions to undertake such data collection, analysis and reporting.
– Sound economic and financial standing assessed by evidence of professional indemnity insurance and audited accounts covering at least three previous financial years.
