Recovery and Physical Rehabilitation in Community Nursing Homes Service to be provided by Nursing Homes for people undertaking recovery and rehabilitation in a Nursing Home.
– To provide an active part of an accessible and effective Recovery and Rehabilitation Service to ensure that adult island residents with physical disabilities receive interventions to enable them to achieve increased functional independence.
– To be able to support the recovery and “step up/”stepdown” phases for people who have recently been acutely ill.
– To reduce dependence on health and social care and enable self-management.
– To facilitate interdisciplinary collaboration and an integrated approach to assessment, treatment, care and discharge planning.
