The current contract for Health and social work services – Framework Lead is projected to end on 23-SEP-19 and may be available for retender. Intermediate Care Beds.
A provision within Redcar and Cleveland for 36 Residential Intermediate Care beds in 2 Lots:
Lot 1 (A) 14 Residential Reablement Beds
(B) 6 Residential EMI recovery and assessment Beds (this may be a provision under Lot 1 or
Lot 2)
Lot 2 (A) 12 General Nursing recovery and assessment Beds
4 General Nursing EMI recovery and assessment Beds
(B) 6 Residential EMI recovery and assessment Beds (this may be a provision under Lot 1
or Lot 2)
The 6 residential EMI beds (in total) could be part of lot 1 or lot 2 but not both.
The service will take people who are ready for discharge from James Cook University hospital and the primary care hospitals as well as people who are living in the community and need support to maintain their independence. The Service will provide the opportunity to step up in order to avoid admission to an acute hospital or a care home or to step down from hospital once medically fit to do so. In addition, the service is utilised as a structured part of a patient’s journey or care pathway. The service is delivered directly to the service users via individualised goal orientated care plans drawn up on admission and monitored daily, with both the service user and their family being involved.
